Attending Physician

Responsibilities

Certify the terminal prognosis. Assess patient needs, manages and prescribes treatment. Direct and approve the plan of care. Coordinate care with the interdisciplinary team.

Registered Nurse / Case Manager

Responsibilities

Assess patient and family needs. Develop the plan of care to meet identified needs. Coordinate team visits and ensure implementation of approved plan of care. Ensure symptom control and pain management. Provide patient/family teaching as needed.

Social Worker

Responsibilities

Assess patient and family’s emotional, social, spiritual and financial needs. Develop a plan of care to meet identified needs. Provide direct counseling or refer patients and families to appropriate community agencies. Provide bereavement support.

Chaplain

Responsibilities

Assess patient and family’s spiritual needs. Develop a plan of care to meet identified bereavement needs. Provide direct counseling. Assist with memorial preparations.

Counselor

Responsibilities

Assess family’s bereavement needs. Develop a plan of care to meet identified bereavement needs. Provide direct grief counseling or refer patients and families to appropriate community agencies. Provide any additional bereavement support.

Hospice Aide

Responsibilities

Provide direct personal care to patient. Provide comfort measures and report identified needs to RN Case Manager. Provide emotional support to patient and family.

Volunteers

Responsibilities

Provide companionship and support to patient and family. Provide needed, non-medical services; letter writing, errands and respite time for family. Provide support at time of death and during bereavement.

Medical Director / Hospice Physician

Responsibilities

Co-certify the terminal prognosis with the attending physician. Lead the interdisciplinary team in the development of the plan of care. Provide consultations to other physicians regarding hospice care.

Music Therapist

Responsibilities

Visit hospice patients and families for initial assessment. Guides the patient in the use of music therapy for improving function and quality of life. Encourages patient to play, listen, or move to music as part of a therapeutic program.
